I hereby waive and forever release All 4 Paws Playcare Inc., its employees, directors, owners, agents, successors and assigns from any and all liability which may arise from the pet(s)’ attendance and participation of services provided by All 4 Paws Playcare Inc., including specifically, but not limited to, any and all injury or damage(s) whatsoever arising during such attendance and participation of services.”
I hereby agree to indemnify and hold harmless All 4 Paws Playcare Inc.; it’s employees, directors, owners, agents, successors and assigns from any and all claims by any member of my family or any other person accompanying me to a function of All 4 Paws Playcare Inc. or while located within the premises of the All 4 Paws Playcare Inc. thereof, as a result of any action by any pet.
I hereby represent that my pet(s) is of good health and has not been ill with any known contagious diseases within the past 30 days.
I recognize that the health of the pet(s) is the owner’s responsibility. I hereby represent that all required vaccinations (Rabies, Bordetella, distemper and parvovirus) are up to date. I will also continue to ensure that the required vaccinations will be kept up to date for as long as my pet(s) attends All 4 Paws Playcare Inc. In addition, I hereby represent that my pets have flea/tick preventative treatments applied regularly.
I further understand and agree that in admitting my pet(s), All 4 Paws Playcare Inc. has relied on my representation that my pet(s) is in good health and has not harmed, shown aggression or threatening behavior towards any other person or any other pet.
I further understand and agree that All 4 Paws Playcare Inc. and their caregivers will not be held liable for any problems that might develop with my pets(s) including, but not limited to sickness, disease, injury, running away and death, provided that reasonable care and precautions are followed.
I understand and agree that any problem that develops with my pet(s) will be treated as deemed best by the caregivers of All 4 Paws Playcare Inc. at their sole discretion and that I assume full financial responsibility for any and all expenses incurred.
All 4 Paws Playcare Inc. fees and packages are non-refundable and non-transferable.
9. I agree that my pet(s) may be photographed or videotaped, and those images may be shared on social media and/or for marketing.
All 4 Paws Playcare Inc. reserves the right to permanently remove a pet from daycare/boarding at any time to ensure the safety of other pets as well as staff.
I understand that the rules above apply to any pets(s) of mine attending daycare/boarding and/or grooming.

Clients are to remain in the entrance area only. Should you go in any other area, we are not liable in any manner whatsoever.All pets must have a fitted collar with name tag and contact info for your pet. We also recommend you have a City license tag.We accept pets of all ages once they have received all their required vaccines.
All pets require a behavioral assessment prior to attending All 4 Paws Playcare Inc.All pets are required to be on leash when entering and leaving the premises.All newly homed pets must have been in the home for at least two weeks prior to attending All 4 Paws Playcare Inc.
We accept puppies if they are fully vaccinated and spayed and neutered.All pets must be spayed or neutered to attend All 4 Paws Playcare Inc.
All pets require up to date vaccinations for Rabies, DH2PP, and Bordetella.
Proof of vaccination is required prior to first day, with a minimum of two-week incubation period after administration.
Pets that are currently ill or injured are not permitted at All 4 Paws Playcare Inc. Pets who have recovered from a communicable illness within the past 30 days must provide veterinary clearance to return to All 4 Paws Playcare Inc. Owners certify by signing this application that their pet is in good health.
If your pets incur veterinary bills during their stay at All 4 Paws Playcare Inc., you, the legal owner of the animal, must pay those bills at time of pickup. Pets reserves the right to seek veterinary care for your pet at its discretion.
All pets must be non-aggressive and not food or toy possessive. Owners certify by signing this application that their pet has no history of threatening or harming any person or animal.
All 4 Paws Playcare Inc. Supplies Kirkland Dog and Cat food. If this isn’t acceptable, you will be required to bring adequate food for your pet. We do encourage to maintain their everyday diet, Please put their food in ziplocks and label their name, date and time of feedings with the appropriate amount given in each bag.
All 4 Paws Playcare Inc. Is not responsible if your pet(s) are a flight risk, if you feel your pet(s) is capable to dig under fence, chew through or jump over, please do not attend our facility as we are NOT responsible.
We reserve the right to refuse any pet for any reason at any time.
Pick-ups must be completed prior to 6:00 PM for All 4 Paws Playcare Inc. unless arrangements are made in advance outside of the normal pick-up time. Late fees for late pick-ups will apply.
All pets must be on time for your appointments as our staffing is scheduled based on appointments booking times. Please be mindful of our hours of operation. Late charges are $10/30min (per pet)
All fees must be paid at drop off and before pet can leave the premises.
All 4 Paws Playcare Inc. passes or gift cards have no cash value and are non-refundable. No exceptions.
